When it rains, the grass and top layer of dirt disappear.

Need help in getting it replaced and stopping it from happening. Can anyone give me step by step directions? a book that lays out the steps, a video or a class to take? H E L P
Grass is receeding at the top of picture
The lawn has decreased 4" - 6" below the driveway, which needs to be repalced as well.

I'd loosen the soil or add soil, rake it flat spread grass seed, and cover it with landscape burlap. Water lightly each day. Seed will sprout thru the burlap, then it will rot away. A small bit of work but the results will be worth it. Or have sod put down...faster results but more expensive.
