How can I replace bumpy floor dividers?

Anybody have a solution to replace bumpy, wood floor dividers? I hate that every time you move furniture from room to room you trip over the dividers. I like the look (defining where the wood floor meets the tiles or carpet), but would like something flat, easy to apply and looking good. Thanks

You can see if you can find flatter transition molding,, but it will still be proud of the floor. Flooring has different heights and the transition molding is made to cover the seams, heights for a clean look.
