You need to countersink the nail heads and then fill the void with putty. After priming there shouldn't be any more issues with the rust bleeding thru the paint.
If you haven't primed or painted the nail heads yet you can gather more information about oil based verses water based. We chemically treated the rust and then primed ours with oil based spray primer, as was req'd by rust resolving product. Then we stained with a water based stain and later found out that galvinized metals don't like to be painted with oil based paints. It seems to have some merit since our rust has begun to come thru the white stain.
