How can paint/stain that is flaking be removed?

Nails keep popping up. Ruins sander belt. Old wood, not flat surface.

Hammer boards just beyond the popped up nail. Remove it and replace with galvanized screw. Then you can use a sander. Also, the screws are stronger.
