Why does my zucchini plant turn brown and die?

I started my zucchini from seed in the spring and when it was warm enough (Mothers Day - I live in NY) I transplanted it outside. It grew strong with lush foliage and lots of flowers. Soon after the zucchini started growing the plants showed brown spots and over time the plant died. I carefully watered from the soil so as not to wet the foliage. I am at a loss to understand why it happened. This is not the first time- it seems to happen each year so if I am lucky I get one zucchini from the plant before it succumbs. Any ideas about what's going on?
