Fall, but only the spend plant and any dead limbs. Do not cut back new grow or eyelets or you will not have flowers next year. Hydrangeas bloom on old growth.

Not until the spring if you are in a cold climate. The dead growth will help protect it from frost & snow,

In spring cut back to new growth, just above emerging buds. Cut out all dead branches.

If you really must cut back now then make sure you cover it in horticultural fleece during winter.

    Hope that helps
