I've put in Pergo before and it's very easy and can go over just about anything. You would have to make sure your ceramic tile is perfectly level and in most cases that's an impossible outcome for tile. If you have uneven tile it will affect the way the ergo will lay and cause it to buckle.
You can, but I wouldn't. Too much of an elevation change and creates an awkward transition and tripping hazard.

Was new baseboarding installed to accommodate the new height of the flooring, and how are the transition areas, and any adjoining doorways managed?
