    I guess it depends on how much you want to spend, and how much work you want to put in to it! Paint, wallpaper, stencils are cool; cabinetry for above the sink, new lighting, a sink installed into an old dresser makeover. Looks like you could use more storage too.

    The first question I have is: Do you like the color of the tile? I really like the color of your tile, but you may hate it. Is the tile in good shape or are their broken or severely cracked tiles? If you hate the color or have seriously damaged tiles, then clearly it will need to be replaced which can be tough on the budget. If you do gut the bathroom, will you be doing the work or will you hire a contractor to do the work. Personally, I draw the line at removing and laying tile feeling a pro is the best option. Your bathroom has personality because of the existing tiles. There is limited space so I think any cabinetry will create a more closed-in feeling. I would replace the shower curtain with a clear sliding shower door which vary in price depending on what you select. I'd keep the storage tower over the john and change out the lighting and vanity cabinet above the sink. Keep all of the colors other than your tile in light reflecting tones: white would be my choice but you could go with a very pale pale pale gray or tan or, if you like it better, a vertically soft striped wallpaper to give the room lift and draw the eye upward. I'd also keep the ceiling bright white to reflect light and help the room feel a bit larger. Also keeping the towels and other accessories in the lighter tones won't detract and have the visual effect of 'popping' off the wall. If you stencil, which I wouldn't, I'd go with 'barely there' images. Too much pattern in small rooms can make it feel like a box. Hope this helps!
