Patio floods when it rains

Our patio floods every time it rains and I need to know how we can fix it to make the water drain out into the yard. We have tried several different things but nothing seems to work.

It sounds like a grading issue. Unfortunately, it may mean further than DIY application because you would need to grade the landscape away from the base of the home (downwards) so the water can seep further down naturally and not puddle up on the patio/base of home, causing more water damage.
