You could make the whole bedspread ruffles. You could make a quilted top and pleated skirt or a ruffled skirt you could make the top with a quilted inside and put covered buttons on the top and make a pleated or a box skirt.

I just use the flat sheet as a bedspread, full on twin / king on queen. It is light enough to sleep under and easier to launder than a heavier spread.

To make a sheet look like a bedspread is all in the trim, using a 2 flat sheets you could make a nice bedspread that is lightweight and perfectly matched 1 for the top of the bed and the second one as the ruffle, you could use lace on the edge as well taking it away from looking like a sheet.
