    you can get quotes from local professionals by using the internet - i.e. "thumbtack.com" or "HomeAdvisor.com" (I use them ALL the time)

    Depends on the size of your home and what you want done i.e. floors and other materials needed . Get referals for local contractors who are licensed and insured . Check references. Get more than two or three quotes .

    That's an impossible question to answer. It will depend on your specific house, the materials it is made of, the structural work needed, your roofing, the area you live in, labor costs, architectural expenses for someone to design a project like that, etc. I am sure it will be very expensive.

    Calculate the entire square footage you will need. Mulitply by local square foot building costs. (you can go on line with your zip code to get local estimates) A rebuild will cost more than new construction as you have tear down/demolition costs as well. $150 sq. foot would be US average new cost build. if you want a new bath or kitchen in that, double it.
