Call a professional. Falling from a ladder is life threatening. Don't do it.
HIRE A HANDY MAN
I just get on a ladder and scoop out whatever is in there, then wash wath a giant sponge and dawn dish soap and hose down. Mine do not get too bad, most is dirt and dust from wind. Make sure you clear all downspouts, especially if you have a lot of leaves and consider gutter guards. Here are some links.
