Try bleach. If that doesn't work, try a pumice stone.

The first thing I would try is a heavy duty pumice stone sold by Pumie. You can find it very cheaply on Amazon or at your hardware store.

Mix oxygen bleach (not chlorine bleach) with hot water. When it cools down, flush the toilet and turn off the water. Fill the bowl with the oxygen bleach mixture and let sit for several hours.

If doesn't work, use 8% hydrogen peroxide and soak it for several hours. Then heat the solution to 180-200 degrees and soak the bowl. Rinse thoroughly.
