How can I add hooks to cement plaques so that I can hang them?

You can use an epoxy for concrete. Read to see if it's for anchoring metal to concrete. Most hardware stores or big box stores carry a variety of these.
E6000 glue, or better yet, Gorilla Glue. How heavy are the plaques? This would determine the adherence method and hook material that would be best. Depending on the size, weight, and style, there are ways of attaching ribbon, twine, hemp, etc. to use as another way to hang them, instead of hooks.
Knowing what the plaques are like (weight, dimensions, etc.), where they’re to be hung and on what type of surface, and the style of the area in which they’re to be hung all are factors to consider before knowing the best advice to give.

If you are fearful of them falling see if a plate hanger will work. These are available at hard ware stores.
I have actually painted over wallpaper - 1st coat of kilz primer, then 2nd or third coat as needed to cover print on wallpaper - and YES, worked great, was very easy and lasted for a lot of years (no peeling either).
Use epoxy made for adhering things to cement walls, I would do this and give it a trial run with something non-breakable to see if it will do the job.
