Mine has both mattress & box spring. I like the extra support. Downside? I have to use a step stool to get into bed! The whole set up is about 3-3.5 feet tall; I'm 5'2".
You can do it either way, it all depends on how high you don't mind the bed being.
you can do without but it might make the bed too low. in that case you can buy a "Bunkie board" which is anywhere from 2" to 4" thick. It goes under the mattress instead of a spring.
