Yes you can you have to add unibond or similar to your mortar mix for extra adhesion as I live in Spain this practice is quite common, in fact we can get a ready mix just for tile on tile. I would imagine that you probably have a few different products in your local hardware store.

yes , you can leave them down and go over them. It would be a very tough job to rip out. Don’t forget it will change the height and you will have to cut door bottom and molding.
Yes, just clean the old tiles first. It will look best if you use a THIN glass tile the kind made of tiny squares on a mesh. You can cut the tile say in 4 inch strip you get three 4 inch wide strips which works great with a 4 inch tile. I did this to my 1960 bath it had 4 in. light gray tiles in perfect condition. I just wanted an extra touch. Before you grout the glued glass tiles, Use blue painter's tape along all the edges, will make the grout look neat at the top and bottom
John has a good point about the height of the floor being changed if you were to add an additional layer over top the existing floor. The transition piece between the bathroom floor and the, I am guessing the hallway(?) will also have to be changed and probably rebuilt so that it is not a tripping hazard.
