I would clean them first ,then use a primer or paint with primer.

TSP is the best. We helped friends clean a home previously occupied by HEAVY smokers. There were even lighter squares on the walls where pictures were hanging. When washing Carmel colored drips just went down the walls. It was disgusting. We scrubbed everything. They tore out carpet and repainted with success.

Wash with TSP. It comes in a box at the paint department of most home improvement stores. It's a wonderful product!
