If it is just because it is old, you could re-surface with Slabs or overlay with a Deck. or Crazy Paving etc.
From what I can see, you may have a water problem and that is causing it to crack. It looks like the patio is lower by the house, which is a problem for more than just the cement. Before you fix the patio, you have to deal with the cause. You don't want water running toward the house, you want it draining away. Unfortunately, the way to fix it right is to remove the patio and regrade the ground under it to slope away from the house. You may want to have a mason come check it out and see what he suggests.
Looks like it's cracked, raising, and shifting. Any kind of a repair will just be a band aid. May be time to replace it.
