Just sand the rust off the spots. Clean your register well. I used a satin finish, a couple of coats. Easy to clean now!

Sand and paint with rustoleum or tremclad metal paint
Or you could get rid of it. We put toe kick heaters in our bathrooms under the vanity. They're great and you get rid of those nasty heating runs. No matter what we did, they would just rust again. Here is a basic search so that you can research them. Good luck.

Try scrubbing the rusted areas with a brillo soap pad. Then wipe with a wet cloth to rinse.
Sand it down with fine grit sandpaper.
