Depends on the use of the area -- 20 Mule Team Borax swept into the cracks in the concrete will kill the grass.. One option is for free plans.... It all depends on your skill level and on your budget.... you could also rim the area with plants in containers for interest and privacy.. the horse troughs are great to use for "pony wall" planters or to hold small trees like citrus
Kill the grass,and resurface the pad. A pergola would look great. Concrete footings and posts at every corner and build up.
Build a pergola and use camouflage netting for the top - it's very popular these days for hiding things like compost bins etc. Certainly easier to get these days. Then you can concentrate on planting up some shade-giving plants. Grapes are very popular due to their foliage, just make sure their roots are well sheltered.

I would kill that grass cracking your patio and repair the concrete before you have to replace it.
can you have someone build you a small pergola ? or a Corner One...
