I would clean the tile with a cleaner degreaser, such as Purple Power from WalMart or go to an automotive store and buy a degreaser. If you still wanted to paint after cleaning, lightly sand and apply a good primer that blocks stains. Then paint with BM Advance or SW Poly acrylic. with those paints a sealer is not necessary.
Scrub the dirty areas with TSP. Then I would keep it as is.
I would clean the mantle really good with TSP as mentioned above, lightly sand the wood and paint it a clean white. Also if you really want your fireplace to "pop" you could paint a beautiful stencil pattern on the ceramic tile.
First, try removing the smoke with a Mr. Clean Magic Eraser. Then paint it black to match the rest of the fireplace. Black is a great color to liven up any room if it is used as an accent.
