Clean the walls as best as you can. I would prep the walls, use a couple of coats of something like Kilz that would keep the stain and odor from coming through, then paint the walls. If the walls are stained, most likely the ceiling is too.

What ever cleaning product you use, nicotine is a sticky substance, it can be absorbed by skin contact. Wear gloves...weather they are the dishwashing type or the disposable plastic type. I found a sponge mop with a long handle worked well to clean the walls. Try a dish soap first, as if you are doing dishes. Rinse often.

Vinegar and water will do just fine. You may need to do it more than once though.
