Do you start at the wall or in the middle of the room

I actually start in the center. I first dry fit the tile on the floor with one row in each direction. I move the tiles left to right so I have even cut tiles at the walls. Then I mark the center of the floor. Start laying the tile from the marked center. You don't want any sliver cuts at the walls.

I have done it both ways. I prefer to start in the center of the door that leads into the room. But step 1 should either be to snap chalk lines leading from the door all the way to the wall facing you. Then I start making my way across the room. Most definitel, buy the small spacers because they will help you maintain equal gaps between tiles.
