How can one repair or replace tiles and grout in the bath?

We have a few times that is loosing the grout around them what is the best way to repair ?

Remove the grout and re-grout.
You can buy small amounts of grout for repair in a squeeze tube. Getting it to match color may be a challenge. Clear area vigorously with toothbrush and toothpaste first. Rinse. Dry.
Before putting any new grout, be sure you've removed any old grout.
