You need to install transition molding in the gap.
I agree, you molding that will fit the widest part of the gap, that way you will be able to cover it all with one piece.

As an incredibly lazy person, I’d check out the tile Trim department and find the appropriate size flat trim to fit. A tone of your door or darker than your floor with dark grout.
I reconize that whatever that “base” is would have to be scraped out to depth, but it could possibly easier than getting wood trim, and more endurable.
I just checked my front door and they used quarter round wood moulding. We are on a concrete slab and we have the same white tile and dark gray grout as you do. Hope this helps.
We must have had the same person tile our houses! Quarter round is too small did not fill gap anything bigger was also too high. I tried a few other things too and nothing worked really well. Next I'm trying some wood paint stirrers cut to fit probably use construction adhesive to attach to cement slab if I find they look ok after they are stained they seem to be about the height of the tile. Too bad they did not even leave one piece of tile & could not find any at stores,habitat for humanity resale outlet even been checking local Goodwill stores. I might try to use a piece from under bathroom vanity or laundry room then cut it in strips to fill it in.Let me know what solution you get & if any of mine work I'll try to post a pic of what I do.
