What is your foundation made of? Cement? If so buy a commercial premade cement and fill the holes.
2dogal offers some good advice. I would only add that if there is a large tree nearby the roots may have caused the crack/hole. If this is the case the roots will need to be dealt with before fixing the holes.
Don't use regular cement to fill the holes. Will not hold or last. Holes and cracks in concrete foundations need to be filled with hydraulic cement. It gets hot and expands as it hardens to fill the cracks/holes.
