sand and small pieces of dirt can go thought the rug, especially if it's 100% cotton / 100% wool.

A few times a year (I do it every 3 months or so) turn over the rug, and vacuum under. Leave the rug upside-down for a few hours and step on it. The pieces that got stuck, will come out. Then, turn it over again, vacuum the floor where the rug was, and then vacuum the rug again. It will be as good as new for a few months.

That's the dirt that seeps down that the vacuum cleaner doesn't suck up.

Either dust/dirt build up under the rug or the rug is old and the backing is coming off.
