I would have someone help you do some raised gardens. These images are literally for your situation. With a bit of work, there is nothing we cannot overcome. Good luck.

Do you have a walker. I use mine to get around to tend flowers. So hard to walk on the grass alone so the walker steady's me. If you can not use a walker then I would suggest being very careful on lawns. Sorry I could not be of more help.
My brother-in-law is an avid gardener. However, he has fairly advanced MS and has trouble walking. They have a big garden which is very long.
He has a small motorized scooter and manages it beautifully. He even is able to put a watering can full of water on it when needed! I know they are expensive, but if there is any way you can get one I am sure it will change your life!! It has made a significant difference to both him and my sister, especially when they go out.
Its me again! By the way, they have placed a few garden chairs and benches in the garden which helps if he gets tired or just wants to sit and enjoy the garden!
Have someone help you by putting down wood between the rows & around the garden so the wheel chair doesn't get stuck. My dad always put down old carpet strips between the rows (you can do outside edges too) so you hardly have to weed. I hope these help you.
