This old plate has no screws. Just the two holes on top and bottom. Is there a special tool to get it off using the little holes?

Use the slot at the bottom to pry the face plate off. Inside will be screws that hold the frame to the wall.
You can use a small flat-head screwdriver or flat thin blade knife
It is a bit confusing. The silver caps that are protruding will they unscrew more? Once they are out of the way. You should be able to pop the face plate off with a small flat head screw driver or knife. The holes at the top were for wires to go into the connector if they were not embedded in the wall.
I would use some nippers!
