We also used the Sika premade concrete and that seemed to help too! I'm sorry, this has not happened to me so I don't have more info on preventing recurring cracks, other than to keep replacing with the Quikrete/Sika.

Remove & replace., As in sledge hammer. You can buy premise concrete steps.
if you think the crumbling can be stabilized, then you can install a frost proof non-slip ceramic tiles over them.
More you can build a deck over them, keeping in mind your door threhold height and using pressure treated lumber/coated screws.
