Line the tank with thin sheets of Styrofoam.
we use to have a toilet that was low profile on the tank and looked really nice. except the condensation filled a bath towel daily.. nothing seemed to work, after 2 years of fighting this problem. Replace the floor and purchased a new middle grade $ toilet and no more sweating puddles. we do get a very little tank condensation but nothing to create puddles.
