I am guessing the shelf moves too easy. It should be sitting on a 1x1 piece of wood on each side. Get two more and nail to each side on the top of the shelf. Then it is like slid into those two rails. Then it can not lift up only pull out.
You could also run a bead of carpenters glue on top of the side boards, re-install the shelf and put some weight on it to dry.
If warped, flip or turn over.
Nail or glue it back down.
It could be replaced if other suggestions don't work.
