Ceiling medallions are affordable, can be custom painted, and some come in sections so you don’t have to remove the fan to install. Make sure the center is open enough for your fan canopy. Use construction adhesive to apply. Another option is to either stencil a design or apply a decal, with or without a frame at the outer edge. Textured ceilings might make that option tricky. You could make or buy a very large frame to apply to the ceiling and paint the area between frame and fan a color to go with your decor.

if you go to Lowes or Home Depot they have medallion that goes around the top then you put your light fixture up .
