Letting hydrogen peroxide sit on the grout for at least an hour will help.

Lowes or Home Depot have grout cleaner that works good however,
It the grout is real bad, get a grout file, The grout file will remove
ugly grout nicely. Then re-grout with new grout. After the new grout
dries, apply grout sealer. Follow all instrustions.

Grout sealer goes a long way in preventing mildew growth to begin with.
