As I was pruning one of my Knock Out rose bush, I notices it has grown a stem filled with thorns and the leaves are bushy instead of 5 leaf structure. What does this mean? Is it good or bad for the plant? Is there something I need to ensure that plant will remain healthy and yield roses.

I would indeed consult a master gardener or university to be sure, rule in or rule out. What you are describing sounds like Rose Rosette (Witches Broom). Here's is a reference with a picture (which you can enlarge) to help you, but for sure get this checked out soon.

I would call a local nursery to get the best advice. To me that just sounds like new growth and should mature into what the rest of the plant looks like.
Sometimes these designer plants are grafted onto a hardier plant - what you may be seeing is growth from the plant your roses are grafted onto. I'd just prune that part off. :)
Sounds like new growth. Let it grow a bit and see what happens as long as it looks healthy. It's always easy enough to chop off later.

Sounds like that is part of the Wild Rose Stock Root. Cut it right out as low as you can.
