Refinishing A Mid-Century Modern Dresser: Mistakes & Lessons

Let me start by saying I am beyond happy with how this Mid-Century Modern dresser turned out. (Except for that leg that still needs to be replaced. Don't worry it's on order.) The wood grain is stunning and the stain color from General Finishes Java Gel Stain is exactly what I wanted. But OMG was it a painful process.
Here's the "BEFORE" pic. It wasn't in bad shape but there were a lot of dings and scratches that needed attention.
Worst of all, this ugly laminate top.
So I started the refinishing process. Nothing out of the ordinary, just your standard strip, sand, and stain. Everything looked great until I put on my top coat. That's when these horrible mystery spots appeared!
I asked everyone I knew for help and spent hours online looking for solutions. I even tried using the same products on another dresser to see what happened. More spots!
What was I doing wrong?
