Set it up so that it is convenient for you. It's all common sense. Pen and pencil holder close to phone with message pad. I have 2 oversized monitors at eye level and, yes, 2 keyboards. The rest is up to you. I experimented with different set up configurations every time I made an office move. Here are a couple sites that discuss ergonomics.

Get a paper holder one for in and one for out then get a pen holder with paper clip tape holder small clock go to office max for organizers
Throw away anything you can. It's easier to organize when you have less clutter.
