Since the mantle is such an integral part of the room, you need to select pieces that MEAN something to your family, not just put something up because it looks good or like something out of a magazine or showroom. It should be personal. Something you love to look at or something that will stimulate conversation and make you feel good.

That said, select one or two large pieces, put it slightly off-center (centered is boring!) and work from there. Use a single theme and use an odd number of items since your brain always tries to pair and even number of things up. Simpler is usually better - cluttered is not only confusing, but a dust collector.

My mantel piece is visible upon entering our house. I love to decorate it and have done so over 15 times a year in the 46 years we have lived here. Sometimes the decorating includes the wall over the mantel. I do not agree with choosing something that means a lot to your family, cause most males could care less. Mine designs usually reflect the season, and my own design philosophy. But also my designs go with the style of mantelpiece I have and these can vary hugely from home to home.
