If you want to keep the same look to it, you could do a whitewash effect where you dilute white paint with water and paint the sign. You could dilute the paint alot and that way you would get a nice white antiqued look over the piece and be able to see the image and words through it. Because the surface looks shiny and a bit varnished, you might dilute chalk paint, which will adhere to the surface well without any sanding needed. Just an idea from my brain, I've never tried whitewashing yet!
You could leave it as it is, and just clean and wax it. You could add a color wash to it. If you want to keep what's already painted on it, I would just clean it and wax it.
It looks to me like a tavern sign so you could put your last name and add an apostrophe S. "tavern" You could hang keys from it, put a mirror in the center, Put a basket with a flat back to hold papers.
I would sand it down, and choose a tole pattern for it, personalize it. Easy peesy.
I would distress it a little more. Take it outside and put it face down in gravel, then jump on it a few times until it has more dents and scratches. Bring it back in and use some black paint on an old rag and wipe over the face of the sign so that the paint goes down into the dents, then wipe most of it off and let it dry. Wax with paste wax when dry.
I would just leave it like it is, and put the clear coat over it. Then, yes use those little screw hook's to hold six set's of key's at the bottom.
