Exactly WHERE do you prune the dead flower stems - at the base of the flower, leaving the 'hip', below the 'hip', or at the bottom most part of the flower stem (pruning new growth under the flower too)?

I always cut just above a cluster of leaves.

Lower down the stem, just above a leaf. A new shoot will come out where the leaf meets the stem and you will get more blooms.
Take the branch back to the next node.
Its usually suggested to cut back to a 5 leaf branch on the stem.
