Lime, and a lot of it will get rid of it. Moss loves acid soil.
Is your lawn over clay soil, meaning that it hold moisture? Moss grows also in damp areas. You may need to change the lawn elevation slightly for better drainage.
You can use a product to kill the moss then rake it out.
