Squash has male and female flowers, the male comes out first, then the female. If you don't have pollinators in the area when they bloom, you may not get produce. You could try hand pollinating the female flowers with the pollen from the male flowers. As to the plant dying early, it could be anything from over watering, under watering, to disease or bugs or even soil nutrition.
