Do I have to sand stain off before painting

    You need to sand the surface up, put a coat of primer on and then paint. You don't need to completely get rid of the stain before painting, just make sure it's sanded enough that the primer and paint will adhere to it.

    Not necessarily. It depends on the type of paint you are using, what you are painting, and if there is a clear coat or not. Most new "chalk paints" don't require anything but you should do a light sanding and cleaning prior to painting. If you have a varnish, you will have to remove it. Try a small corner of what you are painting, and see how the area looks. Hope this helps.

    No, but do sand off any varnish, shellac or coating and clean with alcohol And a lint free tack cloth. If stain is dark, you might want to use a primer first or paint that includes a primer in it.
