The roof should not be insulated unless your remodeling the attic into living space. Insulation in betweem and over the joists only. The rafters need air circulation to stay dry. Insulation can trap moisture from the air and mold or rot will set in.
First you need to know what type and how much as well as "R" rating which differs across the country. You also need to check your local Building codes. Many are on line, if not contact your local permit office. This will get you started.

Staple up styrofoam venting between the rafters then staple up paper backed fiberglass insulation. Do this whether you are finishing the space or leaving open.
You can blow in loose insulation between joists or rolls of fiberglass insulation.

I'm not sure where you live, but generally speaking, you would not insulate the sheathing under your roof if your attic is not a conditioned space as William pointed out. You do need what Kathy mentioned, which are styrofoam baffles that insure you are getting sufficient airflow from your soffits to your ridge vent. They have channels that allow the air to flow. This will keep your roof cold and prevent ice dams and moisture related mold growth if you live in an area that experiences cold winters. You should have a double layer of insulation on your attic floor and seal all penetrations from light fixtures, plumbing, exhaust ducts, etc. into your attic.

Foam insulation stops air infiltration, doesn't harbor mold, and still insulates if it happens to get damp/wet.
