You have to lay plastic down and a thin layer of sand down first or u will have cracks come through

Not a good idea. You will have a weak foundation.

This is a bad idea. When I had the patio at the back of my house replaced we discovered a previous pour underneath. The last owners decided to pour a larger patio right over the old cracked one. Result was a new patio with exactly the same cracks as the old one. Remove the pad and have the surface properly excavated, filled with stone, and compacted before pouring the new concrete. Make sure they use metal reinforcement in the new slab. Only costs a few pennies more to go first class.

NO! New concrete will not adhere to old concrete. You don't need to replace like Ken says unless it's really falling apart. You can resurface using a latex modified concrete resurfacing compound.
