Your nearest hardware store has caulking for concrete. Alas, the repair is only temporary. Make sure when you have the new concrete installed that they use plenty of reinforcing steel to keep it from breaking apart.
Chances are you have an erosion issue, and rain and/or snow is not diverted from the area. I would get bids from local contractors in order to properly assess the situation and see what they suggest.
There’s a fairly new process that they have this machine that will squirt a leveling liquid that’s something like insulation in a can that expands to bring foundations, driveways, sidewalks, etc back to its original position! It’s been advertised on TV for months now! Might want to check into that
