You would first need to kill the root or remove the tree, then remove the section of concrete that has heaved up, and then pour a new concrete slab to patch the driveway.

The roots have to be dug out and the concrete replaced. Unfortunately that won't entirely solve the problem because the roots will grow again. The only possible thing that I can think of to solve the issue is to completely dig out the trees or bushes that are the cause and be sure you get all of the roots. This may too much to DIY, so you might need to call a professional.

My first thought is that this is a job for a professional. Only a pro will be able to actually see your driveway and determine the extent of damage and whether full replacement will be necessary. As long as the trees are still there they will continue to change the driveway so any repairs will be temporary. You will likely come to the same conclusion.

This MAY help , as I have never actually done it. AFTER the roots are removed and the concrete replaced, dig a trench along the side of the concrete at the depth of the roots, and introduce copper sulfate mix in that trench. The roots cannot grow through it, or at least I am pretty sure.
