Old shower faucets do not have a thermostatic mixing valve in them. Newer shower faucets do and are required by codes to have them. If your not experienced with plumbing you would need a plumber.
Installing a new mixing valve in the shower can reduce or eliminate the temperature fluctuations. Thermostatic mixing valves automatically balance the amount of hot and cold water being mixed, which will prevent drastic fluctuations in shower temperature. If the cold water flow is reduced (due to a toilet flush), the valve automatically adjusts the amount of hot water being mixed. This keeps the shower temperature more consistent, even when other fixtures are using water.

You could install a small tankless water heater in the bathroom. They run between $150 - $150.
Tell your wife not to run water while you're in the shower.
Heed William's advice. The newer faucets are amazing at dealing with this problem. Make sure you purchase a newer faucet that has this capability.
