Because they have a needle in them, I think they have to go in a sharps container to be disposed of.
I agree with Ann it seems to me that because they have a needle in them they need to be disposed of properly.
Maybe spray paint them and write herb names on them with paint pens for herb markers?

Unless you remove the needles they need to be disposed of properly but if you remove them there are many projects to look into:

I would not repurpose the pens since they have a needle in them and they cannot be recycled they are mean to be disposed of in a sharpies container or other methods whatever state you live in
For anyone not familiar with what an insulin pen looks like, here are some examples. They are not the same as a biro/ballpoint pen.
Hi do you have a photo so we know what they look like? Without seeing them I would like to suggest gluing them to a wreath form and then painting the whole wreath. Add a bow when it is finished.
Once the needle is removed, pen devices can be safely disposed of in household trash. These devices should not be recycled. Vials and Cartridges: Empty insulin vials and cartridges can be disposed of in household trash.
