I saw one of the TV DIY guys do this on a vintage tinted concrete walkway that had been jacked up by tree roots (DIY networks "Restored") they dug out under the walkway and cut out the tree roots.... an expert said that since the tree was large.. removing one or two roots would not hurt it. After cutting out the roots... they dug away as much soil as needed to re-level the concrete... pushed gravel and soil into the cavity... and mostly jumped on it until it settled back down.
